Transcript Requests
High School Transcripts
All Creekside High School transcript requests must be processed through Parchment. There is a $5 fee for each transcript request.

We are unable to email transcripts directly to students, colleges and universities, the NCAA, coaches, or any
other individual or organization. All official transcript requests must be submitted through Parchment.
Creekside transcripts can be requested as a paper copy that you can pick up in the front office. Please allow up to 5 business days for a paper copy of a transcript.
SJR State Transcripts
Dual enrollment transcripts must be requested directly from St. Johns River State College, not Creekside High School.

SAT Score Reports
Sign in to your College Board account, open the Send SAT Scores page, select each college or scholarship
program, choose which scores to send, and review and submit your order. Colleges generally require official SAT
scores to come directly from College Board.

ACT Score Reports
Sign in to your MyACT account, select the score reports option from your dashboard, choose the test date or
superscore and recipients, and complete the order. ACT sends the official report directly to each institution.

AP Score Reports
If a college requests official AP scores, order the report through your College Board account. The report generally
includes your full AP score history unless you request that a score be withheld or canceled.

Cambridge AS & A Level/AICE Transcripts
Students who need official Cambridge AS & A Level exam grades or AICE Diploma status sent to a U.S. college or
university must submit an online request through Cambridge’s Grade Transcript Service. Students will need
identifying information, exam series details, their Cambridge center information, and an accepted form of
identification.
